  • Select the type of request from the available options (access, correction, deletion, etc.)

  • Upload proof of identity (required under APPI)

  • Indicate your preferred response method (email, post, or telephone)

 

Once we receive your request, we will verify your identity and respond within the time limits required by applicable laws (normally within 30 days). We may contact you for additional information if necessary to locate your records. Certain data may be exempt from disclosure under applicable laws, such as information that may harm the rights or safety of others or information retained to comply with legal obligations.
